Our Diabetic Medical Supply Company Accepts

• Medicare - Participating Provider and we accept Medicare assignment
• Private Insurance/Major Medical/Group Health Plans: Accept most plans in all 50 states with the exception of HMO's.

Upon receipt and verification of private insurance or Medicare diabetic benefits, Support Plus Medical will contact you to let you know if our diabetic medical supply company works with your insurance plan and if there will be any cost to you. Please note: since we are a provider with most insurance plans, the majority of our patients have no out of pocket expenses at all! Upon receipt of the completed Medical Authorization form and the Physician's Order from your doctor we will ship your first 90-day supply to your home. Following, Support Plus Medical will contact you once every 90 days to discuss your diabetic medical supply needs before shipping your next order.

Diabetic Supplies Provided:

The types of diabetic supplies that will be sent to you are based on the diabetic supplies requested, your physician's consent and your insurance coverage. If there are diabetic supplies requested that are not covered by your insurance, you will be contacted by our office prior to any diabetic supplies being shipped. For our Medicare patients, please note that Medicare diabetic benefits do not cover the cost of prescription items, but they do cover diabetic testing supplies.
